Engagement and Initial Screenings (Contact within 24 hours)
Our Case Managers contact the claimant within 24 hours of receiving the
file and meet the claimant at his/her home within seven days for an
initial medical review.

Assessment (Medical or Vocational Interview)
We identify expected outcomes and establish rehabilitation goals. At
this point, our Case Manager also obtains a signed consent for release
of medical information and medical records are reviewed.

Planning (Includes estimates and negotiating of price)
Next our Case Managers develop a comprehensive plan of care. In order to
promote cost savings measures we solicit estimates and negotiate prices
for therapy services, supplies, attendant care, transportation, and
durable medical equipment as well as other items.

4. |
Implementation (Incorporating the involvement of Physicians, Therapists, and other professionals)
Involvement of the physician and therapist is important in promoting
medical and rehabilitation stability. Our Case Managers work closely
with these professionals to effectively intervene in the plan of care.
In order to obtain a return to work as quickly as possible, we maintain
contact with the employer to establish the work policy.

Evaluation and Refocusing (Re-evaluation of the plan)
Constant re-evaluation allows smooth transition along the continuum
towards optimal recovery. Plans and interventions are constantly under
refinement for each individual case. Follow though is the key to our
success.

Monitoring (Shared medical visits, therapy progress)
We provide continuous monitoring of patient progress through shared
physician visits, and consulting with the other medical professionals
involved in the case. We communicate the patient's individual needs and
progress to you through telephone contact and written reports.

Disengagement (A return to productivity as early as the first office visit)
We foster a return to work productivity as early as the first physician
office visit. We are organized and adaptable to each new situation. We
promote early intervention, leading to quick case resolution, which in
turn leads to lowering the cost of each case.
